Parental Agreement Form

Dear Parent/Guardian,

In an effort to serve high school seniors in the Chicago-land area, Summit United is providing a distance-based, virtual mentoring program for students applying to college. Summit United scholars will meet with organizational staff and mentors who will facilitate program activities through online platforms such as Zoom, Google Hangouts, and Slack.

This form seeks consent for your child to take part in the distance-based, virtual Summit United program.

We will use the online applications (Zoom, Google Hangouts, and Slack) for the following program activities:

● One-on-one meetings with a mentor who is a current college student or recent graduate;
Note: All of our mentors have completed background checks.

● Workshops facilitated by our leadership team relating to the college application process, applying for financial aid, and professional development;

● Communicate with other high school seniors and mentors in our program using the communication platform, Slack.

Summit United will actively monitor member activity on all platforms used and will make every effort to protect member information and ensure that all activities online comply with Summit United’s safety policies and scholar & mentor code of conduct.
